Several factors converge. Diagnostic criteria were historically developed from studies of young boys with more visible support needs, leaving adults who present differently unrecognized. Many autistic people — particularly women — develop effective masking strategies early, consciously imitating social behavior in ways that hide autistic traits from observers. Individual traits often get explained as personality characteristics, and co-occurring conditions like anxiety, depression, or ADHD are frequently diagnosed first, effectively closing the inquiry.
